教授ら救急医４人全員が辞職

「新医師研修制度」の問題ではなく、地方の国立大学附属病院のありかたの問題ではないでしょうか。

国の政策にのっとり、地方の附属病院はすべて廃止する方向で
現場を汲々とさせ、教授も逃げ出す体制をどう考えるか、
そういう問題だと思います。

「新医師研修制度」とはまったく関係ない話だと思います。

＞時間外の軽症患者（年間約１万２千人）
月に１０００人すなわち１日３０人以上の時間外の軽症患者がいれば夜は寝れませんよねｗｗ

週に１回の当直でも翌日休みをくれないのでしょうから１０回もやらされたら辞めるのが当然かと(大笑)
自業自得と言っていいですね。あ、管理者も辞めるのか。

小生なら３０歳代でも、お馬鹿な勤務させられる事業所は即刻辞めます。

に関しては、「４月から他科の医師約１０人が交代でセンターに入るため」「他科で対応してきた時間外の軽症患者（年間約１万２千人）もセンターで受け入れる」
ということではないかと。
今までは「年間９００人の患者」だったようです。
つまり一日２人以上の「軽症ではない」患者を受け入れていたと言うことですが・・・

国のシミュレーションはある意味正解だったんですよ。
新臨床研修医制度導入の目的のひとつは，医局講座制の破壊とそれによる教授人事権の消滅だったわけで，鳥取大学では，それが見事にかなったわけです。

……医局消滅と教授の辞任が目的ではなかったとは思いますし，副作用が強すぎたといった方が正解でしょうが。
まぁ，今更元には戻れません。

医局講座制の崩壊は国民の望んだ方向です。
その結果としての地方基幹病院の崩壊も国民の……。

次は学徒出陣がご希望のようですし。

新臨床研修医制度導入の目的のひとつは大学医局の人事権を削ぐことだったんでしょうが、別に国民が望んだことでは無いでしょう。
という以前に医局がどういう働きをしていたかも知られていなかったでしょうし。
学徒出陣も希望してないと思います。
自分や自分の家族は研修医じゃなくてベテランに担当して欲しいと思ってる人が多い印象。

要は厚労省の方針でしょう。
医師を根無し草にしておけば配置がやりやすくなると思ったのでは？
医局のしがらみの無くなった医者がさらに扱いにくいものだとは思ってなかったという誤算。

そういえば、阪大の救急の教授もこの3月で辞めちゃいますね。ちがったかな（誤報でしたら許してください）。

教授と言うと、医者の出世の頂点と言うイメージがありましたが、こと救急に関して言うと過去のイメージになってしまいました。「やってられない」というやつでしょうか。

　予算は減らされる、人も集まらない…。
　公務員社会と似ています。公務員人気はあっても、↑の後段は「人も減らされる」ですが＿|￣|Ｏ

医局がなくなったと思ったら、無政府状態。

下手に手を出すと放置していた違法な就労状態で返り討ちにあう状況です。
奴隷労働を前提にして成り立っていた、「安い・早い・上手い」救急医療なんてのは、もう過去の話です。

そのうち勤務医の労働闘争が始まることでしょう。
安田講堂のようなバカなことは必要ありません。
粛々と、生存権に裏打ちされた労基法の遵守を守らせるだけです

研修医制度が始まってから、無理やり一次救急を外病院で担わされていた研修医が拒否し始めました。
一次救急が潰れた結果、二次救急が疲弊し、二次でのしわ寄せが三次救急へと波及して、三次救急が夜間診療所状態。

根本の勤務医労働問題を解決しない限り、『偽装宿直対応』での救急外来は潰れる運命といえましょう。

救急に携わっている自分が言うのもなんですが、救急って、医師にとっても病院にとっても歓迎されざる部署なんですよね。いつなんどき、どういう病態の患者が来るかわからないなかで、万全の診療をすることが期待されていますし、なにか問題が生じれば、マスコミの報道被害や法的な責任を強く問われます。

若い医師はおもしろがって救急に来ますが、いいところ数年で他科に転向します。肉体的精神的に辛すぎますからね。

鳥取大学も研修医を募集する大学病院ですから、救急科はないとこまるのですが、だからといって、あまりバリバリ救急をやられても、コストの問題やら人的資源の工面やら、問題が山積してくるわけです。

今回辞職することにした教授は、きっとはりきってバリバリと救急診療をしようと思っていたんでしょうが、大学側の「まぁ、ほどほどにしてくれよ」という空気が読めなかったのかもしれませんね。

要するに、救急関係の設備の優先度が低いんですよ。ウチもそうですが、院内での発言力や予算獲得力は、収益と比例しますからねぇ。

次期教授がどのような救急の舵取りをするのか、興味があります。

教授や助教授まで辞めていく現状を厚労省はいったいどう責任取るつもりなんですかね。どうせ誰も責任なんて取らないんだろうけど。
医局が労働基準法違反だろとなんだろうと、義務感と責任だけでとどめていた堰を何にも考えないで潰したのでもう戻れませんけどね。

千丈の堤も蟻の穴より崩れる、とはよく言ったものですね。

　制度改正前の時点で、全国８０の大学病院本院の内でおよそ２０は潰れてもらうというのが厚労省による非公式の目論見だと、当初から言われていました。

　ですから、地方国立大学病院における医療崩壊は、決して予想外に発生した問題点ではなく、ほぼ予想通りの結果ということになります。

　まさにその通りですね。小生は医局は円満退職させて頂いたので未だに医局OBづらしておりますけど、好き勝手なこと語っています(^^;)。
　最近は大学医局入局者はホントに少ないようでどこも大変です。いわゆるジッツの維持が出来ないのです。教授といえども今となっては人事で無理強い出来ませんね。医局員に辞められるくらいなら自治体やマスコミなんか糞食らえと思っておられるでしょう。

医局というと　悪弊のみを　指摘されますが，
わたしのような古い人間は　どこどこの教室の出身
ということをきいて　どれくらいの技量とか
どちらのベクトルを向いて育てられたとかを
推量していたのですが，
それがなくなると　まるで　人物評価ができない
君らの都合じゃんということなかれ，
これは従来の医局の持っていた教育システムを
各人は反映することを前提にして評価していたことを
意味するわけで．どこの馬の骨かわからん人を
情報なしに一から評価するのは
至難の業ですよ．まあ　どこの業界もおなじでしょうが．
いままでは担保する組織があったのに
なくなった　ということで

フランスでは今年から全国民、主治医を指定登録することになって、留学生であるうちの馬鹿息子まで登録させられた、登録医は本人が決めることになっていて、登録すると登録医の指示がないかぎり大学病院や専門病院には初診受付できない。もちろん登録医は任意で変えられる。
これって素人にはとってもわかりやすいんだけど、どうして日本はできないのかなぁ。。。

大学病院は、病院という特殊性はあると雖も、大学組織の一部に他ならない。となると、教育機関・研究機関である「大学という組織体」を所管する文部科学省はどう考えるのか？気になるところです。国立大学の附属病院と雖も、組織体としては文部科学省の所管である国立大学法人（が設置する国立大学）に包含されるかたちになりますから。

今の保険点数のままでは、赤字補填能力のない地方の大学附属病院は、大学本体から切り離されてしまうかもしれませんね。その結果、赤字倒産、となってしまうのでしょうか。

（いま、ちょっと離れたところにいるのですが、)自分の所属する医局も、すでに崩壊が始まっていました。。。大学に戻るのが、怖いです。
どこも同じで、タダ働きと奴隷奉公の日々、、、
