医療アラーム「聞き逃し」多発、救命遅れで死亡３０件（2009年3月19日03時07分 読売新聞）
アラームは人手不足の医療現場で、患者の急変を知るために広く使われている。患者に１日着けると原則１５００円の診療報酬が出る。調査を担当した永池京子常任理事は、「装着の必要性や異常を知らせる設定値をよく考えないまま着けておくという医師も多い。このため『無駄鳴り』が増え、看護師が鈍感になり、本当の警告音を聞き逃すことにつながっている」と指摘している。協会では、「アラームの聞き逃しが看護師の不注意や怠慢のように言われてきた。調査結果は氷山の一角で国を挙げた取り組みが必要」と対策を提言する。提言は、〈１〉医療機器取り扱いの専門職である臨床工学技士（ＭＥ）の増員〈２〉医療の質を重視した診療報酬への見直し〈３〉看護師の医療機器に関する教育〈４〉聞き逃し事故の情報を共有するシステムの構築――を提言する。
アラームを着けていて、聞き逃した、あるいは放置したら救命が遅れたということは、つまり、アラームを着けていたことに必然性があったということだと思います。
オオカミ少年の話を知らないまま、育った人もいるでしょうが、アラームを放置する看護師は、その患者一人一人の状態を把握しないまま、看護しているということになるように思います。
いかにも医師が、何も考えずに着けているから悪いという意図が伝わってくるけれども、必要最低限の医学知識があるはずの看護師が、モニターの意味もわからないまま仕事をしているとしたら、国家資格に対する責任感や自負なんてどこにもないということになりはしないだろうかと思います。モニターのついている患者さんのアラームが鳴った時くらい、見に行くのは当然の仕事なのですから。
そうなんです。仰るとおりなんです。
ただ、解決にあたっては複合的な問題として取り扱う必要があります。
あちこちの病院にお邪魔していると、常態として複数のアラームが鳴り続けている病棟なんて珍しくもありません。未だにアラームの細かい設定のできないモニタも珍しくありません。機械的トラブルと生体自体の問題の区別も難しいのでしょうが、技術的問題解決での対応が必要なところは小さくないだろうと思います。
また、見に行けるだけの看護師の数がいないという問題も出てきます。一時にできることは誰でも一つです。看護師が一度に見に行くことのできる患者さんも一人です。患者と看護師が常時１対１でもない限り、病棟で同時に二つのアラームが鳴って、どちらも致命的問題で手が離せなくなるという事態が生じることを確率ゼロにはできません。
着けた医者が悪いとか、見に行かない看護師が悪いとか言っていても、患者のリスクは減りません。
また、いずれにせよ際限なくコストをかけることが許されるわけもありません。アラームによって命が救われる僥倖もありますが、いつもというわけにはいかないという諦めも必要と思います。
現在のアラームは非効率過ぎるのです．かつてのように単体のモニターを別個に付けていた時代と違い，今は統合型モニターですから，個々の計測値を総合してアラームを鳴らすように改良すべきなのです．今のアラームではかなりの部分がfalse positiveですから，人手不足の現状では全てに対応することは困難です．
統合されたアラーム，いわゆるインテリジェントアラーム（もしくはスマートアラーム）の構想は既に15年以上前からあるのですが，なかなか実現しません．
24時間モニターを装着して監視しなければいけない状態の患者さんが一般病棟のあちこちに点在している、という状況そのものが間違いなんだと思いますね。
大きな病院がどんどん急性期に特化していくにつれて、従来であればICUに収容していたような患者さんが一般病棟に入るようになってきています。本来であれば、こうした状態の患者さんはナースステーションから一目で見渡せる範囲におくか監視カメラなどで「ナースステーションでアラームが鳴ったらすぐに状態を確認できる」ようにしなければならないのです。「いちいち確認しに行く」体制ではだめ。
でも、こういった体制を作るには病棟改築など多大なコストがかかります。すぐにチェックしに行かなかった看護師さんの責任にしちゃえば簡単 (以下略)。

ICUにするには２床に１名の看護師が常時張り付いている必要がありますから，昨今の看護師不足の状況ではまあ困難でしょう．
せいぜいHCUくらいでしょうか...
いずれにしても医師，看護師を急に増やすことはできませんから先に書いたようなアラームの改良とか，ビデオカメラetc.やネットワークを生かしたモニタリングシステムの改良といったテクノロジーを活用するのが最も早道かと思われます．
従来のようにICUに収容できないのはもちろんなんですが、
ICUやHCUと一般病棟の間を埋めるような構造の病室が必要なんだろうな、ということです。ナースステーションの横に、ステーションから容易に患者さんの様子が確認できるような構造の病室を作って、そこに24時間監視が必要な患者さんを集める、と。
実際、最新の設計の病院ではそうした病室を備えるところがあります。
でも、多くの古い建物の病院では、モニターのアラームを聞いて看護師が廊下を走っていく体制なんですよね。

うちの病院はリカバリーと称する病室がナースステーションの横にあって，そこにはモニターが並び，時にはベンチレータが稼働する，というようになっています．
都内病院勤務医先生が，おっしゃるのはこういった部屋のことですよね．
どんなにインテリジェンスの高い正確なアラームシステムを構築しても、それを受け止めて処置に走るナースや医師の絶対数が不足していれば無意味であると思います。

おっしゃるように絶対的人手不足の解決にはなりませんが，毎回「オオカミ」アラームに振り回され，無駄なエネルギーを消耗させられる愚は軽減されると思います．
同時に複数のトラブルが発生したような場合には人手不足だとどうしようもありませんが，false positiveを減らせることができれば各人の負担は少なくなり，働きやすくなるところが利点と考えています．
人手の問題は人を増やす以外どうしようもありませんが，すぐには実現できません．
