１．医師は、別紙１の症例定義に基づき、新型インフルエンザ（豚インフルエンザウイルスＡ／Ｈ１Ｎ１）の疑似症例と診断した場合には、直ちに最寄りの保健所に報告する。

イ疑似症患者
医師は、38℃以上の発熱又は急性呼吸器症状＊1があり、かつ次のア）イ）ウ）エ）のいずれかに該当する者であって、インフルエンザ迅速診断キットによりA型陽性かつB型陰性となったものを診察した場合、法第１２条第１項の規定による届出を直ちに行わなければならない。

ただし、インフルエンザ迅速診断キットの結果がA型陰性かつB型陰性の場合であっても、医師が臨床的に新型インフルエンザ（豚インフルエンザＨ１Ｎ１）の感染を強く疑う場合には、同様の取り扱いとする。

ア）10日以内に、感染可能期間内＊2にある新型インフルエンザ（豚インフルエンザＨ１Ｎ１）患者と濃厚な接触歴（直接接触したこと又は2メートル以内に接近したことをいう。以下同様。）を有する者

イ）10日以内に、新型インフルエンザ（豚インフルエンザＨ１Ｎ１）に感染しているもしくはその疑いがある動物（豚等）との濃厚な接触歴を有する者

ウ）10日以内に、新型インフルエンザウイルス（豚インフルエンザウイルスＨ１Ｎ１）を含む患者由来の検体に、防御不十分な状況で接触した者、あるいはその疑いがある者

エ）10日以内に、新型インフルエンザが蔓延している国又は地域に滞在もしくは旅行した者

＊1．急性呼吸器症状：
急性呼吸器症状とは、最近になって少なくとも以下の2つ以上の症状を呈した場合をいう

ア）鼻汁もしくは鼻閉

イ）咽頭痛

ウ）咳嗽

エ）発熱または、熱感や悪寒

　この症例定義に疑心暗鬼の方がおられても、不思議で無いと思います。

GW当直中です。

この当直中、何度か発熱の患者が来院しました。幸い、全員インフルエンザ陰性でした。
僕は、発熱患者から問い合わせが来た時には、渡航歴が無くても全員に発熱相談センターに一度問い合わせをしてもらって、一般病院へかかるように指導をしてもらってから来て貰うようにしています。21時以降は相談センターが閉鎖されるようですので、明朝9時まで自宅待機をしてもらい、センターが開いてから電話をしてもらっています。病院を、入院中の患者さんを守るべき立場からすれば当然の行為です。

そもそも、ウチは産科病院で、赤ちゃんや妊婦さんがゴロゴロしている施設です。こういう時期でなくとも感染症の患者が来るような性格じゃ無いんですけどね。。。感染症の患者は、たとえ妊婦のかかりつけであっても内科で対応して欲しいというのが本音です。

　連休中なんです。厚労省の考えている体制は平日昼間だけの話で、連休や夜間については全く考えていなかったと、タダそれだけの話です。

　普段から、大半の病院の当直医は病棟での急変に備えるために院内当直しているだけです。

　やむを得ず外来を受け付けているにしても、特にインフルエンザ対策のために発熱患者を無制限に受け付けるという体制にはなっていません。

　この数年、インフルエンザの流行期にはいつも問題になっていますが、本当にフルの可能性がないのなら、一般の診療所か、せいぜいが急病センターや休日当番医等への受診を勧めればいいはずです。それを休日当番医等ではなく、連休中の一般の手薄な病院への受診を勧めるというのは、万一これがフル患者だった場合に備えての、役人特有の責任回避以外の何ものでもないのでしょう。

　流行期にこんな長期の連休と重なることはないので同情の余地はありますが、立案者と、紹介に当たった発熱相談センターの担当者の誘導が悪かったということです。

　それを病院に責任転嫁するのは、ケンカを売っているのだとしか思えません。…だったら、厚労省や都庁の医系技官や看護系技官が総出で自分たちで診療すればいいのです。数は充分いるはずです。

　だいたい、熱が出れば病院にかからなければならないというのが間違っています。発熱以外に受診の理由がないなら、自宅でたっぷり休養して、充分に水分を取っているのが一番です。風邪は病院では治りません。

　気温と湿度が上がってきましたから、もうあと１〜２週間で今シーズンの騒ぎは終わるでしょう。役所の関係者には、１１月に備えてきちんと考え直してもらう必要があるように思います。

…こんな騒ぎが繰り返されるようなら、現場はいつでも会議室の皆さんに場所を譲りますよ。

中には現場経験のある方もいらっしゃるかも知れませんが、概ねはデスクワーク専門の方々ですから、予算の取り方は知ってても、ラインの取り方も知らない方々には診てほしくないなあ。

そんな「準備」ができる連中なら、医療も健康保険制度もこんな状態になる前にいろいろと「準備」してますわな（ToT）

　　良い機会ですから、国民の皆さんに、「公衆衛生的見地から、アクセスの制限や隔離などの政策を取ることは、行政の責任であり、公共の福祉は個人の権利よりも優先する」ということを、ようく理解していただきたいと思います。　　　
　　マスメディアの皆様、どうぞ、「診療拒否」と騒ぎ立てずに、公共の福祉のためにもっとも正しいと思われることを啓発してくださいますようお願いいたします。

　　以下は想像ですので、間違っていたらごめんなさい。マスメディア、医療機関、患者、それぞれの視点から考えてみました。

推察その①：　マスメディアの一部には、「パンデミックになったら逃げ出すという医者がいる」「診療しないのは、自分が感染するのが怖いからだろう」という偏見があるようです。

　　しかし、これまでに、医師が医業を放棄したという事例が一つでもあったでしょうか。

　　医師が目の前の発熱患者を追い返した、という例が出たら、どうぞ報道してください。

　　休日に、電話での問い合わせで診療できないと答え、しかるべき医療機関に誘導するのは、診療拒否ではないでしょう？

　　医療という限られた社会資源、それも日本の医療はデータ的には世界一級なわけですが、その濫用をたきつけることで、どんな公共の利益があるのでしょうか？それとも、発熱患者はどんどん病院に行きなさい、とでも？

推察その②：　医療機関は、発熱患者が豚インフルエンザでないという保証はどこにもない、とわかっているのではないでしょうか。だから、救急外来で隔離ができない設備の病院では、できれば断りたいと考えるのは当然のことです。重病で入院している患者にインフルエンザが院内感染すれば、それこそ責任が問われますから。

　　でも、監督行政機関である厚生労働省の通達には逆らえませんので、できるだけ穏便なやり方でお断りしているのだろうと思います。

　　（自分が感染したくないから、という理由での診察拒否は、まあ、普通の医学的知識のある医者ではありえないでしょう。医者でなくても、誰でも、人ごみに出かければ感染の可能性はあります。目の前に、豚インフルエンザの発生国への旅行歴はないと自己申告している発熱患者が来れば、接触時間が問診・診察・検査・結果説明その他で最低でも１５分間は生じるでしょうから、その間、グローバル・プリコーションで粛々と診察するだけのことです。）

推察その③：　患者が発熱だけで受診したい理由は何でしょうか？
　　まず考えられるのは、医学的知識がない、という理由です。その責任は、国民に保健教育（風邪は医者にかかってもなおらない、まずは市販の鎮痛解熱薬で経過を見る）をしてこなかった文部科学省・厚生労働省・医師会に、等しくあります。日本の医療費のムダのかなりの部分が、本来医療機関を受診する必要のない「感冒（抗生物質処方の保険病名：急性上気道炎）」です。あまりに広く誤解が浸透しており、これを払拭するには相当な努力が必要です。
　　次に考えられる理由は、何とかタミフルを処方してもらいたい、ということでしょうか。インフルエンザ検査が陰性でも、「親切な」医者なら（あるいは「気の弱い」医者なら懇願または非難すれば）処方してくれるかもしれませんから。

　　これまた、マスメディアに啓発をお願いしたいところです。
　　タミフルが必要な患者家族は厳密な適応（治療・予防）に従って処方されるのであり、供給が限られている薬品ですから、賢く使わないと、長い目で見ると結局自分で自分の首を絞めることになります。

　　医療者も、実際に自分の診た患者が陽性と出るまではタミフルは飲まずに仕事しているのです。あまり早く使い過ぎると耐性になってしまうかもしれないので濫用のリスクもわかっています。

　　検疫官をごらんなさい。業務拒否をした人がいるでしょうか？彼らは通常のインフルエンザワクチンとマスク手袋ガウンという「グローバル・プリコーション」以外、特別な防護はしていません。

　　このことを、マスメディアは冷静に報道してください。

　　そしてどなたさまも、免疫力を維持するため、過労に陥らぬように気をつけませう。

都もいろいろあったので、策は講じているようですし。　あまり地方行政責めても。

都では人口が多く「疑い例」段階で公表すると対象が多すぎて無用な混乱を招く。都の施設で６時間程度で感染の有無が確認でき、国への届け出は感染が確認されてからでも時間に大差はない▽該当者と行動をともにした人に注意を促すなどの初期行動は進めており実害はない－といった理由から、国への届け出と情報公開を見送っている。すでに「数人」が対象になったという。

秋からの季節性のインフルに、協議しておいたほうが。A/H1N1A/Brisbane/59/2007（タミフル耐性　H274Y）の系列の株は必然ですし。

厚労省トップがあれだけパニクっているのをテレビで見たら、国民やマスコミにもっと冷静になれと言っても・・・

パンデミックというのは有事に相当するものです。
通常の患者の権利は抑圧しなければ、多くの市民の命が逆に危うくなります。

航空機が緊急事態になったら、乗客への全てのサービスは止められ、乗務員の指示に従うように命令されます(命令であってお願いではない！)。

これだけ毎日広報活動を行いながら、非常時についての権利抑制について、全く言及しないのは、行政の長としての能力を疑います。

鳥インフルエンザで予測されるパンデミックというのは、そういう事態です。

　業務中の感染が怖くて臨床医は務まりません。ウイルス性肝炎患者の手術にしり込みする者は医学生のうちに淘汰されます。
　しかし新型インフルエンザが怖いから診療拒否をするだろう言うマスコミ人まで危険を冒してまで診る気になれない医師も出てこないとは限りません。臨床医はどのような人間も平等に診療をしなければならないので見当外れの患者でも診療を拒否できないのです。
　でも臨床医の怒りの矛先が医療職行政官に向かっていいでしょうか。稀にみる事態に対して彼らは充分以上の働きぶりだと思えます。前例のない事態に対する決断を非当事者が批難すべきでない思います。事態が鎮静化してから決断を評価することは必要と思いますが進行中の事態に対する指示は不穏当でない限り協力すべきです。

逆でしょう。業務中の感染が怖くない輩は、実際に感染して淘汰されていきます。怖くない者は臨床に向かない者です。
正当に評価すれば、怖いものは怖いのです。
問題は、どうやって対処するかという一点に尽きます。

根性論で突破しようというのは愚の骨頂です。

医療職行政官＝当事者、臨床医＝非当事者みたいな論調だし、当事者(＝医療行政官)に非当事者(＝臨床医)は、従えというような御主張ですが、身内の感染症のプロの厚生技官(木村盛世氏)からもボロクソに言われている愚かな厚労省の処置に黙って従うのは、愚か者かお人好しにすぎる。

第一、協力を要請するにはそれなりのお願いの仕方と、対策費が必要なのだけれど、厚労省は医療従事者の二次被害対策についてのコメントを出しているのを見たことがない。

大学に人を出せと要求するが、コストを提示することもない。

最初のコメントにも表れているように、人の命を救う者の財産・生命を保護する感覚が決定的に欠如しているとしか思えない。

厚労省の行政官が予想以上の働きねえ・・・・
単に、まだ新型ウイルスが入って来ていないだけの今の状況で大騒ぎしている割には、実際にパンデミックが起こった時のための啓蒙番組や啓蒙のための広告が少なすぎるんじゃなかろうか？

有事に相当する医療資源枯渇後の対策が告知されている状況ではないと思うが？？？

不穏当というか不適当な厚労省の通知・政策は、熨斗を付けて丁寧にお返しさせていただくまでのことです。

致死率の高いパンデミックというのは、「どのような人間も平等に診療をしなければならない」限界を超えた事態なのに、何の理解もないのだなぁと、もしNo.11 tatakawa さんが厚労省内部の人間だったら、絶望に近い気持ちになります。

診療を受ける権利を制限してでも、大多数の人の生命と安全を守らなければならないための法整備はできているのか？
と、逆に聞きたくなります。

パンデミック状態では、それこそ「応召義務の例外」宣言をしなければならないはずです。

個々の医療機関ではできないことを、医療行政機関が行わないといけないのに、診療ゴッコをして果てているような行政官は無能を曝け出しているに等しい。

改めて読み返してみると、そうでもなかったです。（汗）

今回の流行で、臨床医が当事者でない、というのはどうも違和感を感じますね。

いずれにせよ眠い頭での書き込みは厳に慎みます。

勝手な推測だけでいろいろ言われてもと思います。
通常でも大学病院で発熱だけという患者さんを受け入れるでしょうか？
また、2次、3次の救急で、かぜ症状だけを訴える患者さんを全員受診させていたらただでさえ救急は忙しいのにパンクです。
今回のインフルエンザがなくてもかぜ症状のみの患者さんで、救急で電話相談されたら、「朝まで待てませんか？」とか「かなりお待たせする事になります。」とやんわりという事はあります。
(飛び込みでいわゆるコンビニ受診はありますが。）

そういって、お話してかぜ症状だけの方で、納得される方もいるし、納得されない方もいますが、一晩で一人、二人はいらっしゃいます。当院の医療圏で15万人ですから、東京では診療拒否されたと感じるひとが、数十人～数百人いてもおかしくはないと思います。

昨年のゴールデンウィークの時には、このようなかぜ症状のみの1次救急で、2次救急で診て貰えなかった人はいなかったのでしょうか？
本当に、診療拒否されたと感じる人の数字は、例年の連休に比べて多かったのでしょうか？そこから比較しないとただ単なる意味のない数でしょう。

国内医療機関における新型インフルエンザ（A/H1N1）診断の流れ Ver.1

新型インフルエンザ（A/H1N1：以下、新型インフルエンザとする）は、2009 年5 月5 日現在、国内での発症者は確認されていない。しかし、検疫通過時点では症状がなく、その後国内で発症する可能性は否定できない。

そこで、Ver.１では、「国内発症者なし」あるいは「発症者が少数」時点での国内医療機関における診断の流れにつき記載するものであり、疑似症例発生に伴う積極的疫学調査ならびに感染拡大防止策に関しては、それぞれのガイドラインを参照のこと。なお、各自治体の体制に応じて、既に診断の流れを決めている場合は、それを優先とする。

要旨

・ 新型インフルエンザを疑う症状を有する患者が電話連絡なく医療機関を受診することがないように、十分に情報を徹底させる。

・ 「都道府県が定める発熱外来あるいは感染症指定医療機関」では、
􀂾 電話連絡があった患者には、受診の時間・受診の入口を指定し、マスク装着の上、他の患者と接触がないように十分注意して、診察室に誘導する。

􀂾 確定検査は、現時点では、原則として、地方衛生研究所で実施されるRT-PCR 法等、新型インフルエンザウイルス遺伝子の検出をもって行うが、適切な検体採取と検体の搬送は極めて重要であるため、十分な準備をしておく（「新型インフルエンザヒト感染例に対する検査診断（医療機関から地方衛生研究所への流れ）」参照）。

􀂾 迅速診断キットは陰性であっても、否定できない場合があるので、詳細については、以下の内容に基づき適切な判断をする。（再検査、再受診の指示など）

・ 「都道府県が定める発熱外来あるいは感染症指定医療機関」以外の医療機関では、􀂾 電話連絡があり、渡航歴や患者との接触歴・症状から新型インフルエンザを疑った患者には、「都道府県が定める発熱外来あるいは感染症指定医療機関」を示し、電話連絡した上で受診するよう伝える。

􀂾 「都道府県が定める発熱外来あるいは感染症指定医療機関」が不明な場合は、最寄りの保健所もしくは発熱相談センターに問いあわせ、電話連絡してきた患者に、適切な医療機関の情報を提供する。

􀂾 電話連絡なく受診した患者で、渡航歴や患者との接触歴・症状から新型インフルエンザを疑った患者については、当該医療機関で診察･検査を実施するのではなく、速やかに「都道府県が定める発熱外来あるいは感染症指定医療機関」に搬送できるよう最大限の援助を行う。

・十分な情報提供をしていても、突然、連絡なしに新型インフルエンザを疑う症状を有する患者が受診する場合を想定し、すべての医療機関で玄関への掲示、受付での症状の確認を徹底させる（感染拡大防御策を講じずに、長い間、待合室で待っていることがないように、十分に注意する）。

国内未発生期における発熱外来を置かない医療機関への発熱患者の受診について
新型インフルエンザ患者の国内発生に備え、関係者との情報共有や発熱外来の設置など、医療体制の確保等について対応いただいているところですが、海外発生期（国内未発生期）における発熱外来を置かない医療機関への発熱患者の受診について、下記の通り、基本的な考え方をまとめましたので、所管の全医療機関にご周知いただきますようお願いいたします。

記

○　まん延国への渡航歴や患者との接触歴が認められる発熱患者が、発熱相談センターを通じずに発熱外来を置かない医療機関を受診したり、電話による相談があった場合には、まず発熱相談センターに電話で相談し、必要に応じて紹介される適切な医療機関を受診するように勧めること。

○　発熱相談センターの指導に従って発熱者が発熱外来を置かない医療機関に受診した場合は、患者にマスク等を使用するように指導するなど、感染予防に必要な指導を行った上で、当該医療機関が診察すること。

まん延国への渡航歴や患者との接触歴が認められる発熱患者は、は、症例定義を鑑みて10日以内に、まん延国への渡航歴や患者との接触歴が認められる発熱患者は、　と思うのですが。

厚生労働省新型インフルエンザ対策推進本部の事務連絡　と、国立感染症研究所感染症情報センターを比べますと、

が、国立感染症研には記載が無いのですが、この場合、厚生労働省の方が拘束力を持つでしょ。　このような範囲が異なる文章が出るのは、どのような場合なのでしょうか。ご教授いただけましたら。

　この事務連絡には、連休中という状況の中で起こったことについての、調査能力（意志？）と状況判断の悪さなど、自分たちのやっていることに対する反省のかけらも感じられません。

　まさにコンビニ受診を助長する政策で、責任回避しか考えていないようにしか感じられません。

この実質命令書で、発熱相談センター及び臨床医も混乱しているようです。　ハイリスクを取り扱う機関からは、異論が。

発熱相談センターも、10日以内に、の記載が有ると無いでは、対応に違いが出るととの事。10日以内の基準を満たす場合は、発熱外来誘導、それ以外は、通常の医療機関へ。　10日以内が関係しないなら、発熱外来へ。発熱相談センターて、診断書面を出しました？。　

まん延国だけにこだわる事に疑問が有るのですが。国立感染症研は、発生は必然の位置に立っていると思うのですが。

連休明けですね。外来にも風邪引きが何人か来ています。インフルは居なかったですね。
まあ、平日に診療拒否するのは困りますが、大型連休は各病院の事情があるので、今回報道されたレベルの内容ではコメントのしようがありません。(悪意に満ちた報道であるのは間違いないけどね)

『しかし、同省新型インフルエンザ対策推進室の高山義浩室長補佐は「発熱外来の病院数が少ない感染拡大期は患者が１日に何十人も押し寄せることはない。夜間救急はしていなくても、当直医師などで十分対応できるのではないか」と指摘している。』

お役所とケンカをしたいわけではありませんが、お役所の現場に対する理解ってこんな程度なんですよね。なんか、ため息しか出ないです。

　高山先生は佐久総合病院総合診療科からの任期付出向者です。

　自分の経験の中だけで語ることの難しさも実感されており、適切な質問をすれば妥当な答が返ってくる人ですから、記者の質問の仕方に問題があるのであろうと思います。

　そう考えて記事を読んでみれば、この答はあくまでも一般論であり、問題となった病院や連休という状況の中での特殊解ではありません。

　記者の意図と記事の書き方に問題があると考えます。

高山氏のコメントが書かれた記事は、発熱外来の設置拒否についてのもので、診察拒否についてのコメントではないのではないでしょうか。連休とか関係ないと思いますが。。。

他の医師の方のコメで「悪意に満ちた報道」
っていう表現も見かけますが、
医師の先生方のほうもマスコミの報道を敵意の色メガネ
を持って読んでる気がしますね。

　前段については同意させていただきます。

医師が、悪意に満ちた報道と認識する理由として、連休前にどう読んでも、怪しい患者は保健所か発熱外来を設置した病院を受診させなさいとしか読めないような文書があちこちで流れていたからです。
やれと言われてやったことなのに、過剰反応だとか、医師法違反だとかコメントする無責任な奴が親分をしている省からの通達がすべての原因なんですけど。
連休前の、病院側の受け止め方として、検査キットやタミフルを統制する目的ではないかと受け止める状況、つまり、それぞれ入手困難な状況がありました。

インフルエンザの検査キット、ウチの病院では底が見えてきました。あと２０人分ぐらいしかありません。
ウチは小児科外来もやっているので、あと１週間もつかどうか分かりません。もちろん、問屋に問い合わせてもいつ入荷できるかは分かりません。
ウチみたいな中小病院・診療所は何処も似たようなものでしょう。そんな武器弾薬が枯渇してきた状況で、どうやって戦えというのでしょうか？

というか、現実的に、今後の方針に頭を抱えています。

インフルエンザの検査キットやタミフルなんてのができたのはほんの数年前、それ以前はそんなものなかったわけですよ。なけりゃないで｢もうありません｣で済む話です。

タミフルにしたって、2日以内に飲めば平均有熱期間が1日短くなったってだけで、これを投与すれば重症化を防ぐなんてデータはどこにもなかったと思います。われわれ医師こそ冷静に対応しましょう。強毒性のウィルスではないのですから。

先生の所では、「もうありません」で患者さんが納得してくれるのですか？
残念ながら、ウチはそういうわけには行きそうにありません。

現在は症例を選んで（インフルエンザの患者に接触歴のある発熱患者に限って検査をするなど）対応していますが、産婦人科という科の性格上、入院させるかどうかを判断する際にインフルエンザかどうかをはっきりさせざるを得ない状況と言うのもそれなりにあるんですよ。

まぁ、無いものは無いので、どうしてもと言う場合は他の施設に紹介せざるを得ませんけどね。

ちなみに、タミフルとかリレンザは、僕は基本的には使用していません。正直に効能をお話しすると、僕の患者さんでは処方を希望しない患者さんの方が多いです。

追記です。

個人的には、水際作戦を潜り抜けて国内発生するのは時間の問題だと思っています。すなわち、いつ自分の目の前に新型インフルエンザの患者さんが出現しても不思議では無いと。
勿論弱毒性なのは承知しています。しかし、病院を守るという視点から、どうしても疑い患者は隔離する必要があります。繰り返しますが、ウチは産婦人科ですから。

そういうわけで、今まではあまりタミフル・リレンザは使用していませんでしたが、今後はA型については積極的に使用をせざるを得ないと思っています。
そういう意味もあって、検査キットが無いのは非常に困るのです。

数年前にインフルエンザが大流行した時には、確かインフルエンザキットがなくなってしまったことがありました。そのときは｢もうないよ｣でおしまいでした。状況からしてインフルエンザが疑わしいときはタミフルを処方してました。
私のところは、タミフルの効用および副作用を話すとなぜか｢のみます｣という患者が今でも多いです。

本音を言うとすれば・・・
弱毒性の今のうちに、自分自身（と家族）は罹患しておきたいです。

そう考えている人も多いんでしょうけどね。

だから、水際作戦などとむなしい努力をしないでさっさと蔓延させてしまったほうが、きっと速く終息すると思いますよ。

このような状況の下、「新型インフルエンザ対策ガイドライン」（平成２１年２月１７日新型インフルエンザ及び鳥インフルエンザに関する関係省庁対策会議）の「医療体制に関するガイドライン」において、「都道府県は、新型インフルエンザ以外の疾患の患者に対する医療を破綻させないため、都道府県の判断により新型インフルエンザの診療を原則行わない医療機関等（例えば透析病院、がん専門病院、産科病院等）を定めることができる」としています。
都道府県は、このように新型インフルエンザの診療を原則行わない医療機関を指定する場合は、一方で、指定されていない医療機関が新型インフルエンザの感染が疑われる患者等を受け入れることとなります。

強毒性に変異した変異株ウイルスが、弱毒性ウイルスの抗原性をそのまま維持しているかどうかは予測できないのでは？
弱毒株の抗原ペプチドに対して獲得された免疫は果たして、強毒株にも有効かどうかは、予測困難でしょう。
やはり罹患しないよう努力すべきだろうと思います。
結果は強毒株がパンデミックを起こしてからではないとわからないと思います。
これが現代医学の限界、レトロスペクティブな科学の限界ではないでしょうか。
もちろんこれを克服すべく世界中の医学研究者は日夜奮闘しているのですが。

なぜ悪意に満ちているかというと、その周知しないといけない通知の対象というのは、まだ無知で放置されてる無関心な一般大衆であって、その一般大衆を受け止める医療機関ではないはずだと思うからです。

絶望とか、怒りというのが、希望と現実とのギャップだとすれば、医療機関と一般市民との意識の乖離があれば、互いに不幸になるだけなので、一般市民への啓蒙こそが、本来やるべき行政の義務であったり、社会の公器であるべきマスコミの仕事だったりするべきと思うからです。

一般大衆の最下層の認識にマスコミが合わせて、社会への理解を求めないやり方というのは、マスコミの商業化、大衆化として批判されるべきものだと思います。
マスコミに啓蒙的要素を期待するからこそ、評価は厳しくなります

その厳しさは、市民が医療に求めるものと同質だと思ってます。

現実問題として、水際作戦で蔓延を阻止することはもう既に不可能であり、それに固執する時期は過ぎたと考えます。
第一に空港での発熱者のスクリーニングも潜伏期間中の患者は当然すり抜けますし、不顕性感染でそのまま入国しウイルスをばらまく可能性も十分あり得ます。
隔離や観察も完全阻止を目指すには余りにも中途半端でお粗末であり、尽力されている現場の方々には失礼かと思いますが、政府のパフォーマンスに踊らされている様にも感じます。
また、ヒトーヒトだけではなく、マガモなどを媒介に国境を越えることも可能性はあると思われます。(既に1978年にHsw1がDuckから分離されたという報告もあり、Hsw1N1のキャリアになる可能性が指摘されています。)
既にアジアでも認められている段階で日本だけが水際作戦が成功し続ける可能性は限りなく低く、肝腎の検査のPCR法も100%正確なわけではありません。
水際阻止作戦が成功し続ける可能性は限りなくゼロに近く、正直言って不可能であり、今後疑い患者が増え、隔離者が増えて行った場合にそれを受け入れる施設や医療スタッフのキャパシティも限界があることを考えねばなりません。

また、確かにおっしゃるとおり弱毒株に対する免疫が強毒株に対して有効か分かりませんが、少なくとも弱毒株に対する免疫は獲得できます。
仮に、中国・東南アジアでH5N1とHsw1N1のハイブリッドが出来たとしても、Hsw1N1に対する免疫が出来ていれば、全くの新型よりは、免疫を獲得している可能性を期待する余地があると思います。

＞致死率の高いパンデミックというのは、「どのような人間も平等に診療をしなければならない」限界を超えた事態なのに、

今回のインフルエンザの騒動は、致死率の高い新型インフルエンザが発生したときの予行演習程度だと思って興味深く拝見しています。
新型インフルエンザ(H5N1)の場合、致死率60%なので診療拒否や医師法違反どころの話ではありません。逃げるのが正解。
それどころか他の患者への感染を避けるため病院は直ちに閉院するのが正しい判断だと思います。
町中にゾンビが徘徊しているような状況を想像します（笑）。

街頭で抗インフルエンザ薬（T705？？）や解熱剤を配るのが一番良いかもしれません。インフルエンザ検査キットもきっと無駄な気がします。発熱相談センターなんて全く何の意味もありません。

そんな中で診療に従事する医療者をしっかりと補償するための予算をつけることが望まれます。

今の厚労省や大臣の言っていること、マスコミの報道を見ているととても不安でなりません。

各自治体毎に少しずつ内容が違うという状況もあるようですが、いっぱひとからげに医療機関を非難する報道姿勢には悪意があるでしょう。
いたずらに社会不安をあおりたてているように思います。

と思うのですが、仮に「悪意」があるとしてどんな
「悪意」なんでしょうかね。

多分、記事を書いた記者は通知の存在を知らなかったんじゃないかと思います。私の感想は「取材不足」で「迂闊」だなあというものです。

一般市民さんの目には、医者がマスメディアを敵視したり厚生労働省のやっていることを非難したりしていることが不可解なのであろうと推測いたします。（間違っていたらごめんなさい）。
　　医者の敵は病気や死ですが、医者は、負け戦を戦っていることを自覚しています。
　　この基本的スタンスを、医者以外の方々と共有できていないことが、医者の悲劇の始まりなのだと思います。少なくとも、日本では、なぜか（皮肉にも、世界一長寿なせいかもしれませんが）、受診して経過が悪いと病勢とは思わず医者がミスをしたせいだと誤解してしまう患者家族が多くなってしまいました。
　　Memento mori.

医者は絶望的な戦いをしているのに、そのことを理解しない患者や一般国民やマスメディアや役人が、共闘するどころか後ろから撃つようなことをしていると感じると、さらに絶望してしまう。そういう感じ方をする医者を、世間は理解できず、さらに叩く。悪循環ですね。

　　例えとして適当かどうかはわかりませんが、男と女の間にある溝は決して埋められないけれど仲良くやっていくっきゃないのと同様、医者と非医者との間にある溝は決して埋められないけれど理解しあえるように努力するしかないのです。双方の歩み寄りが必要です。医者も努力不足なのでしょうが、マスメディアも、諸外国と比べると、日本の医療ジャーナリズムの質には改善の余地がありますね。だいたい、医師会を目の敵にしているという、ひどいバイアスがありますから。

　　今回の豚インフルエンザの話に戻すと、「診療拒否」を報道するニュアンスが、「非難」めいていることに、医者は慨嘆しているわけです。

　　マスメディアは、「発熱だけで医療機関を受診するな」と報道すべきなのです。

　　この有事に、限られた医療資源を叩いてどうするんですか。

　　医者は、自分の生身の心配、家族の心配、そういう個人的なことを抱えながらも、プロフェッショナルとしてできるだけのことはしようという自負ぐらいはあります。ジャーナリストの使命や国民の義務を果たそうという自負がそれぞれの皆さんにあるのと同様に。一致団結しないで、どうするんですか。

　　マスメディアは冷静に総力戦の戦略を国民が理解できるように報道してほしいと切に願います。間違っても「大本営発表」の愚は繰り返さないでください。

Apple now has Rhapsody as an app, which is a great start, but it is currently hampered by the inability to store locally on your iPod, and has a dismal 64kbps bit rate. If this changes, then it will somewhat negate this advantage for the Zune, but the 10 songs per month will still be a big plus in Zune Pass' favor.

The interface is colorful, has more flair, and some cool features like 'Mixview' that let you quickly see related albums, songs, or other users related to what you're listening to. Clicking on one of those will center on that item, and another set of "neighbors" will come into view, allowing you to navigate around exploring by similar artists, songs, or users. Speaking of users, the Zune "Social" is also great fun, letting you find others with shared tastes and becoming friends with them. You then can listen to a playlist created based on an amalgamation of what all your friends are listening to, which is also enjoyable. Those concerned with privacy will be relieved to know you can prevent the public from seeing your personal listening habits if you so choose.

Hands down, Apple's app store wins by a mile. It's a huge selection of all sorts of apps vs a rather sad selection of a handful for Zune. Microsoft has plans, especially in the realm of games, but I'm not sure I'd want to bet on the future if this aspect is important to you. The iPod is a much better choice in that case.
